Be Finally Free provides Enhanced Care Management (ECM) to deliver whole person, compassionate care to those who need it most. With a focus on justice involved individuals, people experiencing homelessness, those with substance use disorders, and individuals living with serious mental illness, ECM connects participants to the support and services that empower long-term success.

Each participant is paired with a dedicated Lead Care Manager (LCM) who creates a personalized care plan tailored to the participant’s needs, goals, and milestones. Services include intensive case management provided at no cost and are supported by our deep network of community partners and our hands-on approach.

With years of experience serving vulnerable populations, Be Finally Free is committed to reducing incarceration, homelessness, relapse, and emergency room visits, while increasing employment and stability in Kern County.
